We currently have an exciting opportunity for a Stop HIV Social Worker to join our Public Health Department based out of New Westminster!
Build on your education and experience as you:
•Provides intensive HIV/HCV case management services to clients noted having acute complex care needs. Completes collaborative assessments and responds by coordinating, monitoring, and frequently evaluating individualized care plans that adress the client's needs. •Provide advanced psycho-social assessment, counselling, referral and advocacy for clients and families living with HIV. •Identify and provide referrals in accordance with established protocols to appropriate resources/services based on clients' needs; ensures follow up to best support client engagement along the continuum of care. •Requests and/or attends clinical case conferences for client reviews, consultation on new referrals and discussion on transition/discharge plans in collaboration with the FH STOP nursing team and other care providers.
To join the organization we require you to have:
•Master's degree in Social Work from an approved school of Social Work •Three (3) years recent, related experience as a Social Worker providing clinical expertise working with marginalized clients with multiple health issues including HIV and mental health and addictions issues •Current full registration with the British Columbia College of Social Workers •Valid B.C. driver's license and access to a personal vehicle for business related purposes.
