located at Bradley Centre, Chilliwack General Hospital
in Chilliwack, British Columbia. Build on your education & career experience as you
•Facilitate all admissions to the facility by liaising with Access, Home Health, and clients. •Conduct admission interviews and tours, issue medical forms, complete required documentation, schedule admissions, and conduct admission conferences for staff. •Assist residents and family members/significant others with financial matters including equipment acquisition, government applications and other matters as needed. •Provide counseling, education and support to residents and family members/significant others from pre-admission to death or discharge, as needed. •Participate in the development, implementation and evaluation of professional standards and practices by providing input to the Professional Practice Chief, Social Worker and through Professional Practice Committee activities. •Participate in continuous quality improvement activities, OH&S and related safety programs, accreditation and regional initiatives. •Bachelor's degree in Social Work from an approved school of Social Work. •One (1) year recent related experience working with the elderly in residential settings. •Current registration with the BC College of Social Workers.
