As of May 15, 2020 in order to mitigate the risk of the transmission of Covid 19 among persons in our care, patients, residents and also staff in our long term care facilities, an order under the Public Health Act was enacted to limit movement of staff between long term care facilities. While this order is in place staff will be limited to employment in only one (1) long term care facility. This vacancy is located at a long term care facility and will be pursuant to this order.

We currently have an exciting opportunity for you to bring your excellent work ethic as a dedicated Social Worker to our Patient & Family Counselling service located at Queen's Park Care Centre in New Westminster, British Columbia.

Build on your education & career experience as you:
    •Conduct social work assessments in order to identify the patient's and family's coping abilities and anticipated needs upon discharge. •Provide clinical counseling services to patients and families regarding identified problems, personal concerns and treatment objectives. •Consult and collaborate with the interdisciplinary teams regarding patient care issues, resources and the discharge planning process. •Provide in-service education to staff through various tools such as presentations and workshops. •Participate in department or program-sponsored research through various methods such as literature searches, administering surveys and analyzing data as required. •Participate in program and community resource development activities by working with community service agencies and the hospital in order to respond to identified gaps in service and recognized patient needs.

To join our team you have:
    •Bachelor's degree in Social Work from an approved school of Social Work. •One (1) year recent related clinical social work experience in an healthcare setting. •Current registration with the BC College of Social Workers.
