is committed to working on issues related to social justice, inequity and empowerment and serve a growing, multicultural population resulting in social work practice that is rich and diverse.This position offers a flexible, rotating schedule which allows time off for work-life balance. Build on your education & career experience as you:
•Deliver comprehensive social worker services and therapeutic clinical counseling to patients suffering acute episodes of mental illness and their families. •Conduct comprehensive social work and psychosocial assessments of clients using a variety of assessment tools and in accordance with professional standards, clinical policies, and practices. •Educate and provide consultation and support to patients and their families, and healthcare professionals on issues pertaining to high risk factors, recovery and current community practice in the field of mental health. •Actively participate in the planning for care during the hospital stay, and facilitate optimal discharge planning by interviewing the patient and/or family to assess and determine a care plan and type of service required. •Communicate/collaborate with mental health and related community service agencies and family physicians by collaborating in the assessment and management of individuals at risk. Provides formal liaison service to the Mental Health Centre by attending the weekly rounds. •Develop, lead, and deliver in-services, presentations, psycho-educational sessions related to mental health To join our team, you will need:
