Social Worker, Hospice Palliative Care

We currently have a fulfilling opportunity for you to bring your excellent work ethic as a dedicated Social Worker within Palliative Care located in the vibrant Tri-cities of British Columbia.

Build on your education & career experience as you:
    •Provide therapeutic counseling services to clients registered in the Hospice Palliative Care Program for an assigned area and to families experiencing complicated circumstances relating to illness or with end-of-life issues. •Conduct social and psychosocial assessments of clients and families and interpret the information using a variety of modalities in accordance with professional standards and evidence-based clinical practice. •Follow-up and provide ongoing monitoring of psychosocial situations; evaluate client's response to interventions and follow clients through the continuum of care in hospital and in the community. •Act as a member of the interdisciplinary care team by providing information regarding client's psychosocial situation and collaborating with colleagues, clients and caregivers to develop a comprehensive plan of care. •Provide information regarding community resources and access to these resources, facilitating referrals where appropriate and facilitating the development of community resources by identifying services needed to support the health of families. •Participate in quality improvement activities by providing input into the development, implementation and evaluation of program goals, objectives, policies and procedures. •Promote client/family autonomy by ensuring their participation in planning, decision-making, and problem-solving.

To join our team, you have:
    •A Master's degree in Social Work from an approved School of Social Work. •Two (2) years' recent, related clinical experience working with palliative clients, or an equivalent combination of education, training and experience. •Current full registration with the BC College of Social Workers and a valid BC Driver's license and access to personal vehicle for business-related purposes.
