committed to working on issues related to social justice, inequity and empowerment and serve a growing, multicultural population resulting in social work practice that is rich and diverse.This position offers a flexible, rotating schedule which allows time off for work-life balance. Build on your education & career experience as you:
•Deliver comprehensive social worker services and therapeutic clinical counseling to Home Health clients and their caregivers. •Conduct comprehensive social work and psychosocial assessments of clients using a variety of assessment tools and in accordance with professional standards, clinical policies, and practices. •Provide consultation to clients and/or their caregivers by sharing resource information, facilitating referrals to other services, and offering telephone support. Facilitate client's utilization of appropriate community services and acts as an advocate on behalf of clients and their families. •Evaluate effectiveness of direct clinical care and education provided to Home Health clients and their caregivers and provide input to the interdisciplinary team regarding changes required to improve efficiency and/or effectiveness or service. To join our team, you will need:
•Bachelor's degree in Social Work •Current registration with the BC College of Social Workers •Three (3) years' recent, related experience working directly with clients and their caregivers in the community •Valid BC Driver's license and access to personal vehicle for business related purposes Take the next step and apply so we can continue the conversation with you.
