•Working in collaboration with the interprofessional team in facilitating patients' hospitalization from admission through discharge •Expediting effective, quality patient care and coordinates timely and sustainable discharge plans •Addressing barriers and providing leadership to achieve optimal outcomes •Being accountable for the development and effectiveness of policies, procedures and standards to support effective care management and discharge planning •Evaluating progress towards desired outcomes to ensure plan of care and services provided are patient-focused, of high quality, efficient and effective. Your background includes:
•Current practicing registration as a Registered Nurse with the British Columbia College of Nursing Professionals (BCCNP) •Three (3) years recent, related acute care experience including one (1) year of experience in community care and discharge planning
This is a casual role located at Surrey Memorial Hospital (SMH) in Surrey, BC. SMH is a 611 acute care bed, University of B.C. affiliated, regional referral and community teaching hospital that has the busiest emergency in B.C. SMH offers medical, surgical, neonatal intensive care, tertiary, ambulatory, cardiology, rehabilitation, as well as 24/7 emergency services with 158,558 visits annually.
