The Role:

Our client, a healthcare organization based in Vancouver, is seeking a Registered Nurse for the position of Manager and Clinical Lead of the Patient Attachment Initiative. This position is full-time and needing to be filled as soon as possible.


Who you are!

You are currently practicing as a Registered Nurse with the College of Registered Nurses in British Columbia. You have a passion for improving patient care and connectivity within the health care system. You are detail oriented, and have excellent communication, organization and problem solving skills.


Where is it based?

Conveniently located in Vancouver and easily accessible by car or transit.



  • Manage a team of staff including all aspects of human resources (performance evaluation, employee development, oversight on individual job descriptions, scheduling, time management, etc.)
  • Work with senior staff to expand various offices and divisions of the organization
  • Assess patient needs and assist in matching patients to appropriate family doctors
  • Gather and review the information of patients referred to determine whether patient is placed with the appropriate clinic
  • Support development and maintenance of partner relationships
  • Provide timely follow up with patients, partners, and family doctors as needed
  • Maintain key service indicators using database system
  • Maintain, manage and store patient records


Must haves:

  • Bachelor of Science in Nursing or an equivalent combination of education, training and experience.
  • Proven management experience, minimum 3 years
  • Current practicing registration as a Registered Nurse with the College of Registered Nurses of British Columbia (CRNBC)
  • 3 years’ experience in Community and Acute Care
  • A good understanding of the available services/intersections between acute and community services in Vancouver’s health care system
  • A solid understanding in prioritizing patient care needs based on clinical information and patient identified needs
  • Proficiency in MS Word, Excel and Outlook
