We currently have an exciting opportunity for a Licensed Practical Nurse, Outreach to join Mental Health & Substance Use Services - Surrey. The LPN Outreach performs assessments and plans, implements and provides personal and nursing care services to clients, both in their homes and in the community, who have a variety of mental health disorders.

Your specific duties will include:
    •Administer oral, subcutaneous and intramuscular medications in accordance with pharmaceutical and organizational policies and procedures. •Assess and monitor patient's actual and potential strengths and limitations including physiological, psychological, socio-cultural and spiritual needs on an ongoing basis. •Provide a supportive environment for the client; contributes to the development and modification of client's plan of care; attends and provides input into interdisciplinary care planning meetings. •Provide nursing care to clients, both in their homes and in the community, according to identified policies and procedures, technology and Fraser Health standards; evaluates outcomes, records observations and reports problems and/or changes to designated staff. •Assist clients with activities of daily living such as serving meals and feeding, client transports, bathing, dressing, skin care, toileting needs; oversees client exercise routines, as applicable. •Participate in quality improvement and program activities to maintain consistent level of care and to ensure that interdisciplinary team members work collaboratively. •Maintain a safe working environment for the patient and others by adhering to established safety policies; reports unsafe conditions and/or malfunctioning equipment. •Assist in the orientation and education of new staff, students and other members of the interdisciplinary team, in collaboration with the Manager or designate.

Qualifications:
    •Graduation from a recognized program of Practical Nursing or an equivalent combination of education, training and experience. •Current First Aid, CPR, and Food Safe certificates. •Current practicing registration as a LPN with the British Columbia College of Nursing and Midwives (BCCNM). •Valid BC Driver's License and access to a personal vehicle with appropriate insurance coverage.

You will take a thorough, hands-on approach to deliver high quality and precise work. A focused approach is needed to complete routine day to day tasks and handle changing calendars, priorities, and timelines. You will carry a high level of integrity, discretion, and confidentiality.
