Position Summary

In accordance with the established vision and values of the organization, the LPN Home Support Team Leader/Primary Care Nurse, coordinates Home Support Services by supervising Community Health Workers, overseeing the implementation of client care plans, providing training and evaluation of personal care techniques, provides advocacy services, and also functions as a member of the interprofessional team and applies best practice to provide competent, safe, and ethical care for people and their families in settings such as physician practices, homes, schools, and communities.

The Home Support Team Leader/Primary Care Licensed Practical Nurse performs assessments, plans, and implements and provides personal and nursing care to people. The LPN administers oral, subcutaneous, and intramuscular medications in accordance with pharmaceutical and organizational policies and procedures. The scope or extent of the skills that the LPN performs varies according to the predictability of the outcome for a given population. The LPN operates in accordance with the competency guidelines and full scope of practice within the Standards of Practice as outlined by BC College of Nurses and Midwives (BCCNM) and according to organizational policies, procedures, standards, and unit specific protocols.

Shift Rotation/Hours of work: 07:00 to 15:00, 15:00 to 23:00, 23:00 to 07:00 Days, Evenings, Nights (Rotating)

Qualfications
•Registration with BC College of Nurses and Midwives as a practicing LPN registrant.
•Two (2) years of recent, related experience including one (1) year in a supervisory role; or an equivalent combination of training and experience.
•Current valid BC Driver's License.

•Ability to supervise others.
•Demonstrated knowledge and skills within a specific clinical specialty or patient population dependent on the nature of the caseload (e.g. acute care, gerontology, palliative, etc.).
•Ability to communicate effectively, both verbally and in writing.
•Ability to establish and maintain rapport with clients.
•Ability to work independently and collaboratively with others.
•Ability to observe and recognize changes in clients.
•Ability to plan, organize and prioritize; and ability to implement a plan of care.
•Ability to operate related equipment including proficiency in relevant computer applications.
•Physical ability to carry out the duties of the position.
•Use of own vehicle
