program in Fraser Health and support our clients in their psychosocial well-being by facilitating individual and group activities within the community of Hope, BC
. This is your chance to work independently with individuals, providing expert care while executing our Team's care plans.This opportunity will allow you to:
•Provide support to mental health long term care facilities & programs in the community. •Use your Social Services Worker education & experience to effectively execute individual care plans. •Collaborate with Case Managers and interdisciplinary professionals to promote social functioning of clients. To join our team, you will have:
•Grade 12 plus successful completion of the Community Mental Health Worker Certificate or Social Services Worker Diploma and have 2 years' recent related experience, or an equivalent combination of education, training and experience •Valid Class V BC Driver's License and access to a personal vehicle for business related purposes.
