Job Summary:
This temporary fulltime position requires flexibility and responsiveness. Reporting to the Detox Program Supervisor, the Discharge Coordinator (DC) is part of a multidisciplinary team, performing a variety of program related duties in support of clients in inpatient detox services within a harm reduction model. The DC needs to be capable of functioning as a team member, as well as independently. The position will be Monday through Friday, 7.5 hours per day 0730 to 1530 hours.
Key Duties and Responsibilities:
Discharge Planning
- Supports clients in establishing referrals to Social Assistance (SA) and Employment Insurance (EI) benefits
- Supports self-referrals to treatment for clients and where appropriate, referrals to treatment
- Makes referrals for clients to community services as appropriate
Completes Interior Health (IH) Mental Health and Substance Use (MHSU) intake documentation using the IH System as appropriate to transition clients to community supports
Client Education
- Provides “Take Home Naloxone” education to clients and staff as required
- Develop, implement and evaluate programming for clients working collaboratively with existing resources and in conjunction with the detox supervisor
- Facilitates groups for inpatient detox clients, providing one-on-one supportive interactions as required
Other
- Performs other related duties as required as directed by the Detox Supervisor and/or her designate.
Qualifications:
Education
- Minimum of undergraduate degree in social work, human services or related area of study
- At least 2 years training in the field of mental health and substance use
- At least 2 years’ experience in the field of mental health and substance use
- At least 2 years’ experience in providing group and one-on-one support
- At least 2 years’ experience working with community agencies to promote clients’ ongoing treatment & wellness
- Working knowledge of community resources within the IH region
- Knowledge of pathophysiology and psychology related to substance abuse.
- Knowledge of Motivational Interview Techniques and Stages of Change
- Working knowledge of Microsoft Office applications
- Maintains knowledge of developments in the field of chemical dependency withdrawal and treatment
- Proven ability to work as a team member
- Demonstrated ability to communicate effectively with clients, co-workers and people in the community
- Demonstrates a respectful, empathetic, non-judgmental attitude to clients and colleagues.
- Physical ability to carry out the duties of the position
- Ability to organize and prioritize
- Ability to analyze and resolve problems
- Able to effectively lead/facilitate groups
- Able to provide crisis intervention
