Job Summary:

This temporary fulltime position requires flexibility and responsiveness. Reporting to the Detox Program Supervisor, the Discharge Coordinator (DC) is part of a multidisciplinary team, performing a variety of program related duties in support of clients in inpatient detox services within a harm reduction model. The DC needs to be capable of functioning as a team member, as well as independently. The position will be Monday through Friday, 7.5 hours per day 0730 to 1530 hours.


Key Duties and Responsibilities:


Discharge Planning

  • Supports clients in establishing referrals to Social Assistance (SA) and Employment Insurance (EI) benefits
  • Supports self-referrals to treatment for clients and where appropriate, referrals to treatment
  • Makes referrals for clients to community services as appropriate

Completes Interior Health (IH) Mental Health and Substance Use (MHSU) intake documentation using the IH System as appropriate to transition clients to community supports 


Client Education

  • Provides “Take Home Naloxone” education to clients and staff as required
  • Develop, implement and evaluate programming for clients working collaboratively with existing resources and in conjunction with the detox supervisor
  • Facilitates groups for inpatient detox clients, providing one-on-one supportive interactions as required

Other
  • Performs other related duties as required as directed by the Detox Supervisor and/or her designate.

Qualifications:

Education
  • Minimum of undergraduate degree in social work, human services or related area of study
Training
  • At least 2 years training in the field of mental health and substance use
Experience
  • At least 2 years’ experience in the field of mental health and substance use
  • At least 2 years’ experience in providing group and one-on-one support
  • At least 2 years’ experience working with community agencies to promote clients’ ongoing treatment & wellness
Knowledge
  • Working knowledge of community resources within the IH region
  • Knowledge of pathophysiology and psychology related to substance abuse.
  • Knowledge of Motivational Interview Techniques and Stages of Change
  • Working knowledge of Microsoft Office applications
  • Maintains knowledge of developments in the field of chemical dependency  withdrawal and treatment

  • Proven ability to work as a team member
  • Demonstrated ability to communicate effectively with clients, co-workers and people in the community
  • Demonstrates a respectful, empathetic, non-judgmental attitude to clients and colleagues.
  • Physical ability to carry out the duties of the position
  • Ability to organize and prioritize
  • Ability to analyze and resolve problems
  • Able to effectively lead/facilitate groups
  • Able to provide crisis intervention

The Phoenix Centre’s roots were planted in Renfrew County in December 1975. Noticing a lack of support and suitable resources for children with emotional and behavioural challenges and their families, the community united to make a change. A chain of community-based efforts spanning over three years led to the nesting of The Phoenix Centre in 1978, when the Ministry of Community and Social Services confirmed the financial support required to institute a Children’s Mental Health Centre. Since 1978, The Phoenix Centre has grown with the community, offering a warm, safe place to all the children and families of Renfrew County. We currently have 16 full-time therapists and eight child and youth counsellors providing mental health support services to the children, youth, and families of Renfrew County. We strive for your well-being. With our main office located in the heart of beautiful downtown Pembroke, and satellite offices in five communities, our diverse services and programs are accessible throughout the County. Proud neighbours and hosts of the Renfrew County Child Poverty Action Network, Big Brothers Big Sisters Renfrew County and Elevate! Youth Fitness Centre in memory of Krista Johnston, The Phoenix Centre is also affiliated with many local, provincial, and national organizations. Incorporated as Renfrew County Youth Services, our Board of Directors searched for a unique name to identify the agency within the community. The Phoenix Centre was chosen. At The Phoenix Centre for Children and Families, we believe the process of seeking help and change mirrors the flight of the Phoenix. Getting treatment is a challenging process, but, by working through it, children and families can live better and feel stronger than they did before, achieving the extraordinary.
