The Integrated Plan of Care (IPoC) initiative is a multi-year, multi-phase project to support clinical transformation through the creation and enhancement of a patient-centered, integrated electronic health record.

As a Consultant, Clinical Standardization & Integrated Plan of Care you will use your passion for providing consultation leadership and guidance to clinicians, managers and directors regarding clinical standardization, care standard and principles adoption and the development, implementation and evaluation of the Integrated Plan of Care (IPoC) Initiative.

Build on your education and career experience as you:
    •Work to support compliance with clinical practice established through care standard and IPoC implementation plans. Ensure proper governance is established and processes are followed. Develop an education plan, clinical decision support tools, revision of supporting documentation, and a plan to support the various operational teams and physicians impacted by changes to practice. •Oversee and support related projects and initiatives, incorporating change management and knowledge translation considerations. •Oversee clinical standardization and IPoC staff as designated, and related projects as assigned, including the supervision of day to day operational work. •Maximize health human resources utilization by facilitating full scope of practice for all categories of health providers. •Recommend and contribute to the development of new clinical policies and procedures. •Obtain, review and disseminate current information including IPoC governance committee processes, IPoC directives and targets, project plans, education requirements. •Represent Fraser Health and the IPoC initiative at various events including educational forums, committees. •Collaborate and support all IPoC structures and related clinical groups such as subject matter experts' teams, for the primary purpose of practice integration and facilitating consistency in standards of care and point of care documentation.

We would like you to have:
    •Bachelor's degree in health administration, education, or related health field. •Seven (7) years' recent, related clinical and/or education experience in a complex health care environment including project management and leadership experience, or an equivalent combination of education, training and experience. •Current practicing registration with the relevant college or association in British Columbia.
