Reporting to the Director, Clinical Programs as part of the leadership team, and to the Executive Director Professional Practice/Chief Nursing Officer for the professional practice and academic aspects of the role, you will provide leadership in the planning, development and ongoing quality improvement of clinical supports and services for the Palliative Care program. This position will have responsibilities for Burnaby and Tri-Cities be based out of the Burnaby Home Health office Canada Way in Burnaby.
Serving as an expert in the palliative care populations, and as a consultant, educator, systems thinker and researcher, you support and develop best practice and evidence-based outcomes. You participate in developing the overall strategic direction for the Palliative Care portfolio. With a demonstrated focus on home health / palliative practice, care management, skill mix integration, including the promotion of patient family focused care; you will collaborate with Leaders to ensure safe, effective and appropriate client care from a cost effective, quality and safety framework.
As the successful candidate you have a Masters’ degree in Nursing (other graduate degrees may be considered depending on area of specialty), and five years health care experience, including three years recent, related clinical experience.
