Join our Fraser Health Community team in Surrey to support our clinical staff. We are currently recruiting for Casual positions as a Client Services Assistant to support the Primary Community Care Services.

You will be responsible for:
    •Coordinate financial, administrative and contract aspects of client services and care management under the direction of the Manager, Clinical Services or assigned clinical staff. •Participate with health care provider in care planning with client by obtaining required client information from a variety of sources and documenting client needs/wants. •Gather and document information from existing health information systems for all new clients for review by assigned health care professional. •Work with the client and/or caregiver to complete the financial portion of the client's financial assessments and temporary rate reduction requests. •Work with the client/caregiver to document other personal information to complete waiver forms such as sponsorship or residency forms. •Support long term clients to access community or community related service services in accordance with the established care plan; ensures that the assigned health care professional is kept informed and reviews materials such as policy and procedures and clinical decision-making tools to assist with various care management issues.

In this role you have the following qualifications:
    •Diploma in Social or Health Services such as Community Social Services •Two (2) years' related experience working with seniors or adults with physical and/or cognitive disabilities •Valid BC Driver's License with access to a personal vehicle for business purposes
